How can I get a consent or an affidavit done in Switzerland?
In other countries I would go to the police station, take a blank form, write my statement and get it stamped by the police.

You write it but do not sign it. Go to a notary public and sign it in its presence, and swear it truthfulnes. Notary verifies your id, that you have sworn, and stamps the form. Costs around CHF 100, maybe more. (Zurich: CHF 100 for German/ CHF 150 for other languages , Bern CHF 200)

If you need it abroad you might also need an apostille on top. Costs CHF 40 and is available form the cantons chancellery, where the notary is located.
