Well, In Switzerland actually it is common practice to put a photo in a CV. My swiss colleague confirmed that. Also, the recruitment agency I used before also advised me to put photo and personal information as well.
That's only a US thing, AFAIAA, although of course many folk in the UK seem to assume that anything US also applies to them.
And it's ridiculous anyway, as looking at lists of qualifications and/or jobs with their dates will give a pretty good estimate of age anyway. As for looks, well they're going to have to see you sometime.
Well, yes. I stay "still" because like I said years ago when I was recruiting there were few CVs with photos on them. Of course as you say, they were all nice looking. Perhaps the other 85% were too ugly?

When including a photo of yourself in your CV...should it be an actual photograph or a file imbedded photo in the corner on the CV itself (when sending a physical copy).

The embedded file will be fine, just print it out and make sure it looks OK, plain background, head and shoulders.
