"name at birth" = first name (legal name, no nicknames)
"given name" = last name / surname
For example:
(father) Tom Hanks
(mother) Rita Wilson --> let's say named Rita Hanks after marriage
So:
"name of father at birth" = Tom
"given name of father" = Hanks
"name of mother at birth" = Rita
"given name of mother" = Wilson
Right?
I got very confused so filled it in in German. You send them a copy of your ausweiss anyway, so they can check.
"name at birth" = last name / surname
"given name (s) " = first name(s)
For example:
