Anyone know if you have to still fill in passenger attestation forms for the US from Switzerland. And do you need more than one form per family of 2? Already have boarding pass check in, but Swiss app Just asked if you have the right docs for travel( presumably to show later) . Seems a bit confusing . Thanks

I was in the US last week and yes, this form was required and checked (when I checked in at the airport. I can't remember if it was checked in the US)
It's really simple. It's basically two checkboxes, one that you are fully vaccinated and one that you've taken a test.
I believe that it is one form per person.
Yes still required as of about 2-3 weeks ago. How it is checked or enforced is another matter. I didn't realise I had to give it to the airline person before bording, so as I was rushing through the gate trying to pull out papers out of the luggage, I just grabbed what came out and passed it to her and I don't think I had signed it nor filled it all out cause I thought I'd have time on the plane to do that. She was just urgently collecting papers and had no time to look or verify it, so what I gave was surely not complete. But one never knows how strict they will be.
