Translating Steuererklärung to English or French?

Hi,

I need to submit my Swiss tax report (ZH Steuererklärung) to another country's authorities. However they accept only English or French, so I need to find a way to translate my report, or at least the original (empty) form.

Do you know is there a possibility to find somewhere a translation of the original form, or do you know where can I order such translation?

Not sure if this helps, but the Canton provides an official translation of the form fields, although it is separate from the form itself:
