If it's actually for the 10% co-pay the provider made a mistake, that's none of their business, they have no idea if the 10% are already "consumed" or not. The insurance won't care what the provider says as the co-pay is their authority as part of the KVG (KrankenVersicherungsGesetz).
Pay the health insurance only (once it's time), and take it up with the provider directly asking what regulation they base their billing on and what makes them think the co-pay is any of their business.
My guess is they've seen the invoice, but haven't checked the bank account!
I'd give Swica a call (you'll have a contact person mentioned on their invoice) to find out what they need for the two to communicate directly.
Each time they're very quick to send me a refund though. For 10% it doesn't seem worth the hassle to cause issues when it seems Swica are willing to rectify overpayments when they occur.
