I lost my SIM while traveling (bought a local one for data and misplaced my Swiss one). When I got back, I went into a local Salt store who wanted to charge me CHF 49 for a replacement. This is insane to me since it is seriously just a little bit of plastic and a chip. I asked if there was a way in getting a cheaper one but he said "no", but I can try to call them instead.
I called and I got the same response. But when I asked him about a chance to get one for cheaper, he then changed his tune and told me that if I have an online account with them, I can log-in their site and order one for FREE! Unbelievable. I suppose that it wasn't in the interest of the store employee, but surely if this is a Salt policy, he should have shared it with me too. And frankly if it is a Salt policy, they really should not be extorting people to begin with if you have a contract with them. But I digress...
So, if you need a free replacement SIM from Salt, log into your account (top right "My Account"), go to "Usage" and under this category you will see a small wrench on the right which states "more useful functions". This is where you can order a SIM. It takes a few days in the mail but at least it's FREE. Bless the guy who told me.

I also had a similar problem that my sim would only show me "No Service" and told me that the free SIM replacement service was changed and I had to pay 49CHF, which is ridiculously expensive for what it is. I have now ordered a new SIM from the website, hope it comes soon. Thank you again.
Unfortunately it seems like they deactivated the online SIM ordering option, it now shows up with an error. Then when you call to order it again, they charge you 59 francs. Future people might want to try telling them there is a technical issue with the SIM card rather than that it is lost, in order to avoid the fee.
I've had a couple replaced by bringing in the old SIM ... and once I had a new phone, and needed a SIM to fit it, and the person in the store said that I should bring in the old, but they'll let me skip that this time. No charge for any of those cases.
