Versicherungsnummer?

I am filling a medical form and a bit stuck! I have these 3 fields to fill, No. 3 is the only one clear to me.

1. Grundversicherung: Sanitas??

2. Versicherungsnummer: ?????

3. Versichertenkartennummer: 807 _______

On the card, I don't see any "Versicherungsnummer"

I think they mean the AHV number there.

It's either your AHV number (but I don't know why they'd need that), or your Kundennummer, which would seem more applicable. I'd put the Kundennummer - they'll contact you if there is an issue.

it is the two numbders on the left of the blue side - but I do recommend you remove the picture - we now know your name, date of birth, insurance details and can probably very easily track down other info to steal your identity....

Clearly an example photo of an insurance card - not the OPs personal one. Notice the numbers in circles to explain the sections, and several numbers are all zeros.

I would say

Versicherungsnummer: 01509

Because given those two numbers, the insurer is known, and the person being insured is known.

Versicherungsnummer = AHV Nummer

Why they need to know that?

Because it depends where you are registered as cantons need to pay a certain sum for stationary hospital stay. If you are registered in the canton where the hospital is, that canton pays. If not, your home canton will pay for it.

Likely they want the AHV(V for Versicherung) Nr. This is as close as you'll get to like a social security number.

Versicherungsnummer is NOT AHV number. The form already came pre-filled with my AHV number. Yes, they do always need your AHV number too.

Versicherungsnummer = Kundennummer = Something like this = 68.5396.9

In the above card, it is 00.0000.0

That is kennumer the trägers = ID number of the bearer. It seems that is for all Sanitas customers, exactly same as Fabio Bauer

Exactly this. With Swica the Kundennummer is marked as Ins.Pers.No. and I've had to refer to that when talking to them on the phone and filling in forms.
